    • Continuous cardiac monitoring - HF patients are arrhythmia-prone
    • Teach daily weights: call for 2 lb overnight or 5 lb weekly gain - prevents readmission

    5. Evaluation

    By day 3: SpO2 95% on room air, net negative 4 L, lungs clear, patient states 3 warning signs. Outcomes met.
