    How NANDA-I diagnoses are structured

    Every NANDA-I nursing diagnosis has three parts (the PES format):

    • P — Problem: the diagnostic label (e.g., "Impaired gas exchange").
    • E — Etiology: the related factor ("related to alveolar-capillary membrane changes").
    • S — Signs/symptoms: defining characteristics ("as evidenced by SpO₂ 88%, crackles bilaterally").

    Actual diagnoses use all three parts. Risk diagnoses drop the "as evidenced by" — you cannot have evidence of a problem that has not happened yet.

    The four types of nursing diagnoses

    1. Problem-focused (actual): a current human response — "Acute pain related to surgical incision AEB pain rating 8/10."
    2. Risk: vulnerability without evidence yet — "Risk for infection related to invasive line."
